To request a major change into the Health and Exercise Science (HESA) or Health, Physical Education – Teaching (HPET) majors:

1.  Complete the Change of Major form (click to open).
2.  Drop the completed form off at Packer Hall, Room 216 along with any requested   documentation or email the completed form and any requested documentation to wells@tcnj.edu.   Please note that the completion of this form is not a guarantee that you will be accepted into the major.
